I hereby, acting of my own free will and in my own interest, confirm that I have fully read and agree with the terms of this Agreement, in particular I confirm my consent: to the collection, processing and storage of personal data indicated in the registration form; to the inclusion of the provided information in the database of Verkhnedvinsky Butter and Cheese Plant OJSC hereinafter referred to as the Operator; for the purpose of offering me services, new services offered by the Operator, including through direct contacts with me by means of communication indicated by me on our website.

At the same time I have the right to withdraw my consent to use of my personal data by sending a written notice to the Operator at the address: 1 Partizanskaya St., Yanino village, 211622, Vitebsk region. If I received a notice of withdrawal of consent to use my personal data, the Operator stops processing and storage of personal data and destroys the data within 5 (five) working days from the date of receipt of the withdrawal.

I guarantee that the data contained in the registration form is reliable; when providing information, it does not violate the current legislation of the Republic of Belarus, legal rights and interests of third parties.
